YOU ARE HEREBY NOTIFIED that a Petition has been filed in the above-entitled Court by the Montana Department of Public Health and Human Services, Child and Family Services Division (the Department), P.O. Box 1280, Malta, Montana, 59538 requesting termination of the parental rights of the above-named Indian child's possible putative father and for Permanent Legal Custody of the child with the right to consent to the child's adoption be granted to the Department.

NOW, THEREFORE, YOU ARE HEREBY DIRECTED to appear on the 14th day of November 2022, at 2:30 p.m. at the Courtroom of the above entitled Court at the Courthouse, 314 South 2nd Avenue West, in Malta, Phillips County, Montana, then and there to show cause, if any you may have, why your parental rights should not be terminated and why permanent legal custody of the child with the right to consent to the child's adoption should not be granted to the Department. The child was born on October 10, 2019, in Malta, Phillips County, Montana. The birth mother is Mariah Doney. You have the right to be represented by an attorney in these proceedings. If you are unable to afford an attorney, the Court will appoint an attorney to represent you. Your failure to appear at the hearing constitutes a denial of paternity and a denial of your interest in the above-named child, which denial may result, without further notice of this proceeding or any subsequent proceeding, in judgment by default being entered for the relief requested in the petition.
